  1. In a cup or small bowl, warm one cup of the milk, then stir in the cocoa powder until dissolved. In the slow cooker, stir together the cocoa mixture, sweetened condensed milk, remaining milk, half & half, and vanilla. Cover and cook on low for 2 hours, stirring occasionally.
