Preheat the covered slow cooker on HIGH for 30 minutes.

Put all of the ingredients in the cooker, then pour over 3 tablespoons boiling water and stir until the chocolate has completely dissolved.

Cover the cooker and cook on HIGH for 15 minutes. Serve the sauce hot, or leave on the WARM setting until required.
